Holistic Direct Primary Care provides direct primary care services for adults ages 18 and older in Manatee and Sarasota Counties, Florida. Our services include in-home visits, telehealth appointments, and membership-based primary care. We are a self-pay practice and do not accept insurance, Medicare, or Medicaid.

3. Membership Terms

Membership in Holistic Direct Primary Care is available to adults ages 18 and older. Membership fees are due as specified in your membership agreement. Members may cancel their membership by providing written notice as specified in their membership agreement. Non-member visits are available on a pay-per-visit basis.

4. SMS Text Messaging Program

SMS Program Description: By opting in to our SMS program, you consent to receive text messages from Holistic Direct Primary Care for customer care and account notifications, including appointment reminders, care plan updates, and clinic announcements at the phone number provided.

Message Frequency: Message frequency varies based on your care plan and appointment schedule.
Message & Data Rates: Message and data rates may apply depending on your mobile carrier and plan.
Opt-Out: Reply STOP to any SMS message to unsubscribe from our text messaging program at any time.
Help: Reply HELP to any SMS message or call 941-348-2460 for assistance.
Carriers: Supported carriers include AT&T, Verizon, T-Mobile, Sprint, and other major US carriers. Carrier support is not guaranteed. Carriers are not liable for delayed or undelivered messages.
No Third-Party Sharing: No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.

5. Not a Substitute for Emergency Care

The services provided by Holistic Direct Primary Care are not a substitute for emergency medical care. In the event of a medical emergency, call 911 or go to your nearest emergency room immediately.

6. Limitation of Liability

To the fullest extent permitted by law, Holistic Direct Primary Care shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services or website.
